School Entrance & Immunization Requirements
SCHOOL ENTRANCE IMMUNIZATION AND PHYSICAL REQUIREMENTS
ALL STUDENTS, KINDERGARTEN THROUGH GRADE 12, MUST HAVE IMMUNIZATION PROOF ON FILE TO ATTEND SCHOOL AND BE IN COMPLIANCE TO ALL STATE MANDATES BEFORE ENTERING. WE ARE ASKING THAT PROOF OF ALL REQUESTED IMMUNIZATIONS BE EMAILED OR FAXED TO YOUR SCHOOL NURSE PRIOR TO AUGUST 15TH FOR PROPER ENTRANCE CLEARANCE.
IMMUNIZATIONS ARE DUE BEFORE THE FIRST DAY OF SCHOOL. PLEASE SEND IN IMMUNIZATION PROOF PRIOR TO THE FIRST DAY OF SCHOOL.

MOST PRIVATE HEALTH INSURANCE PLANS COVER THE COST OF RECOMMENDED VACCINES. CHECK WITH YOUR INSURANCE PROVIDER ON YOUR COVERAGE AND WHERE YOU AND YOUR CHILDREN CAN GET VACCINATED.
FOR FAMILIES WITH NO INSURANCE, CHILD HEALTH PLUS, OR MEDICAID, THE WESTCHESTER COUNTY HEALTH DEPARTMENT CAN HELP. CALL THE COUNTY HEALTH DEPARTMENT AT 914-995-5800 TO FIND OUT IF YOU QUALIFY FOR VACCINES AT NO CHARGE.
IF THE DEPARTMENT OF HEALTH CANNOT GIVE YOU AN APPOINTMENT, ASK FOR THE IMMUNIZATION COORDINATOR. IF THE IMMUNIZATION COORDINATOR IS UNABLE TO ASSIST YOU OR IS NOT REACHABLE, PARENTS/GUARDIANS SHOULD CONTACT THE NYS DEPARTMENT OF HEALTH BUREAU OF IMMUNIZATION AT 518-474-1944 OR EMAIL THEM OSAS@HEALTH.STATE.NY.US AND PROVIDE THEIR NAME AND CONTACT INFO. THE NYSDOH WILL FOLLOW UP WITH THE LOCAL HEALTH DEPARTMENT.
FAMILIES WITHOUT HEALTH INSURANCE CAN GET HELP SIGNING UP FOR HEALTH COVERAGE BY CONTACTING A HEALTH DEPARTMENT NAVIGATOR AT (914) 995-6350 OR BY EMAILING HNAV@WESTCHESTERGOV.COM. NAVIGATORS CAN PROVIDE IN-PERSON ASSISTANCE TO RESIDENTS AT 25 LOCATIONS THROUGHOUT WESTCHESTER. SOME AREA HEALTH CENTERS WILL GIVE FREE IMMUNIZATIONS OR CHARGE AN INCOME-BASED FEE SCALE.
PHYSICAL EXAM REQUIREMENTS FOR SCHOOL ATTENDANCE
PHYSICALS ARE MANDATORY FOR ALL NEW ENTRANTS IN GRADES K, 1, 3, 5, 7, 9, AND 11 AND MUST BE SUBMITTED WITHIN 30 DAYS OF SCHOOL ENTRANCE. FOR THE 2022-23 SCHOOL YEAR, PHYSICALS DATED AFTER 9/1/21 WILL BE ACCEPTED. PLEASE SUBMIT A PHYSICAL TO YOUR SCHOOL NURSE DURING THE FIRST WEEK OF OCTOBER.
PHYSICALS PERFORMED ON OR AFTER 7/1/21 MUST BE ON THE REQUIRED NYS HEALTH EXAMINATION OR EHR EQUIVALENT FORM. PLEASE USE THE NYS REQUIRED HEALTH EXAMINATION FORM OR AN ELECTRONIC HEALTH RECORD EQUIVALENT FORM.
SCSD SCHOOL NURSES HIGHLY RECOMMEND THAT YOU SUBMIT YOUR CHILD'S ANNUAL EXAM AND IMMUNIZATION RECORD EACH AND EVERY YEAR. THIS ALLOWS YOUR CHILD TO MEET STATE IMMUNIZATION REQUIREMENTS, TO REMAIN IN SCHOOL, PARTICIPATE IN PHYSICAL EDUCATION AND SPORTS, FULFILL NECESSARY MANDATED SCREENINGS, AND STAY SAFE AND HEALTHY.

THE FOLLOWING SCREENINGS ARE REQUIRED:
- Vision and Hearing Screenings: Grades K, 1, 3, 5, 7 and 11
- Scoliosis Screening: Girls in Grades 5 and 7; Boys in Grade 9
- Distance Vision, Near Vision, Color Perception, and Hearing Screenings: All new students transferring into the district.
* A notification will be sent home only if there are any abnormal findings that need medical follow-up.
A dental exam is recommended during state-required physical exams. Further dental information and neighboring health centers, information can be viewed at Westchester.gov
JV & Varsity Sport Information
Somers Central School District Athletic Department
Somers Central School District offers the convenience of online registration through FamilyID (www.familyid.com). FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for sports; helping us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You will enter your demographic information only once for each family member for all sports. You will still be required to update the health information online prior to each season and the Health Office must have a valid physical on file.
The start date for Somers High School Fall sports is Monday, August 26th, 2024. No student-athlete will be admitted to try out or practice until all mandatory medical information has been satisfied. A valid sports physical must be on file in the Health Office. This physical must be performed by a licensed practitioner within the past year (August 2023 to Present) and must indicate that the student is physically qualified to participate in sports. .

It will be helpful to have the following information handy to allow for accurate completion of your online registration. (example: Doctor information, Health Insurance Information, Student ID)
REGISTRATION PROCESS
A parent/guardian should register by clicking on this Registration Button:
To find your program, click on the link provided by the Organization above and select the registration form under the word Programs.
- Next click on the green Register Now button and scroll, if necessary, to the Create Account/Log In green button. If this is your first time using FamilyID, click Create Account. Click Log In, if you already have a FamilyID account.
- Create your secure FamilyID account by entering the account owner's first and last name (parent/guardian), E-mail address and password.
- Select I Agree to the FamilyID Terms of Service.
- Click Create Account.
- You will receive an email with a link to activate your new account. (If you don’t see the email, check your E-mail filters (spam, junk, etc.).
- Click on the link in your activation e-mail, which will log you in to FamilyID.com
Once in the registration form, complete the information requested.
- All fields with a red * are required to have an answer.
- Click the Save & Continue button when your form is complete.
- Review your registration summary.
- Click the green Submit button. After selecting ‘Submit’, the registration will be complete. You will receive a completion email from FamilyID confirming your registration.

Mission: We, the Somers Central School District School Social Worker, will facilitate collaboration with students, parents, community members, and school staff to identify prevention and intervention services in order to promote student social and emotional wellness.
